Abstract: The patient’s sudden darkening of vision, looking at objects as if through a layer of teal glass, was diagnosed with central plasma retinopathy, also known as central plasma chorioretinopathy, referred to as mesoplasia, a disease that occurs in the macula of the retina and can cause vision loss in the early stages. The patient’s symptoms disappeared on their own after 1 month because of the self-healing tendency of mesoplasm, and recurred after a few months, and was given retinal laser photocoagulation treatment once, and the discomfort disappeared on review, and the vision improved significantly. The patient was treated with retinal laser photocoagulation therapy + oral medication (Ginkgo biloba extract tablets) in March 2021. Three days ago, she woke up in the morning and found that her eyes were uncomfortable, her right eye was darkened and she could see objects as if they were separated by a layer of teal glass. The patient said she was impatient, and after staying up late at work for several days before the onset of the disease, she suddenly noticed a decrease in vision in her right eye and dimness of vision, and thought it was due to fatigue, so she rested at home for 3 days on her own, but the symptoms did not improve, but worsened. On examination, the patient’s visual acuity was 0.2 in the right eye and 0.6 in the left eye. On fundus examination, macular edema was observed in the right eye and the central sulcus reflection disappeared, while the left fundus was normal. In view of the proximity of the macular leakage point to the central recess, the patient was first told that central plasmacytosis is a self-limiting disease and most patients can improve significantly within one month. Ginkgo biloba extract tablets were given to the patient orally, and after 1 month, the patient’s visual acuity recovered to 0.6 on re-examination, and the macular area of the right eye was seen to be fluorescent in the form of dotted flakes on fluorescence angiography, and no leakage was seen, and the patient was very satisfied. However, 6 months later, the patient’s right eye visual acuity decreased again. The patient told us that this time the symptoms were the same as last time, with visual acuity of 0.3 in the right eye and 0.6 in the left eye, with fundus macular edema and loss of central concave emission. The fluorescence angiography showed a leaky spot in the macula, which was far from the central sulcus. The patient was told that laser photocoagulation could be performed because the leaky spot was far from the central sulcus. The patient agreed and underwent laser photocoagulation of the retina once. With these treatment measures, the patient’s visual acuity was significantly improved after 1 week of review. OCT examination showed absorption of subepithelial fluid. The patient’s condition has been controlled after treatment, and the indexes are good. The disease mostly occurs in young and middle-aged people, and most of the patients belong to type A personality, and their temper is easy to be impatient. The patient should be advised to avoid fatigue, rest on time, do not stay up late, and try to avoid mood swings and keep a calm mind. In terms of exercise, you can adhere to exercise, because exercise can make people relax and reduce stress. In terms of diet, pay attention to a light diet, reduce the intake of salt, oil and sugar. Avoid drinking alcohol or alcoholic beverages as much as possible. V. Personal insight Mesoplasmosis is a disease occurring in the macular region. It is currently believed that the onset is related to choroidal hyperperfusion, which increases the osmotic pressure of choroidal tissue, leading to the destruction of the retinal pigment epithelium and the infiltration of fluid under the neuroepithelium, resulting in vision loss and darkening of the patient’s vision. Since most of these patients have type A personality and are prone to impatience, they should be instructed to avoid abnormal mood swings, which can easily lead to the recurrence of mesoplasia, and to avoid staying up late and getting enough sleep, which can also reduce the risk of recurrence of mesoplasia.
