Patient: My grandmother, who is 80 years old, was diagnosed with bile duct cancer last week and it has spread. Due to the obstruction of the bile duct, the bile cannot be removed and jaundice has appeared on her body, but other symptoms are not obvious yet. On June 13, she had an ultrasound and on June 17, she had an MRI and other tests, and was diagnosed with bile duct cancer, which has spread to the pancreas and liver, making cancer treatment surgery impossible. Since my grandmother is old and the cancer is difficult to treat, she planned to have a bile duct drainage surgery to relieve the pain and harm caused by the bile duct blockage. However, I am afraid that her body may not be able to withstand the risky surgery, so I would like to ask you if there is anything else you can do to alleviate the jaundice and other pain caused by the inability of bile flow. Also, do you know if Chinese medicine is effective in treating jaundice? Or is it only supportive? Usually, the methods of reducing jaundice include surgical and non-surgical treatments. If surgical treatment is not tolerated, you can consider PTCD, which is percutaneous transhepatic bile drainage, and requires a specialist in an interventional unit in certain conditions, which is risky but less devastating to the patient than surgery. Preoperatively, it needs to refer to CT or MRCP to decide how many tubes need to be put in. If the site of tumor obstruction is low, stents can be placed to reduce the yellowing, and the tubes can be removed at an optional stage. Regarding TCM treatment of jaundice, one needs to see whether the jaundice is hepatocellular or obstructive. For obstructive jaundice, TCM can usually only play an adjunctive role.
