A specific type of chronic laryngitis

Today we’ll talk about this particular type of chronic laryngitis —– reflux laryngitis. Reflux pharyngitis, also known as pharyngeal reflux, is a disease that has only been widely recognized by otolaryngologists in recent years, but in fact it is very common in clinical work. For a long time, the quality of life of patients is seriously reduced, and some patients even develop depression, anxiety, obsessive-compulsive and other psychological disorders. In recent years, as otorhinolaryngologists have gained a better understanding of laryngopharyngeal reflux, they have found that the incidence of laryngopharyngeal reflux is very high in the population, accounting for 10% of all patients seen in otorhinolaryngology clinics, 50% of patients with hoarseness, and 60% of patients with pharyngeal dissension.  As the name suggests, laryngopharyngeal reflux is caused by the reflux of stomach contents into the throat, which irritates and damages the mucous membrane of the throat and causes the corresponding symptoms. The refluxed material (gastric acid and digestive enzymes) can directly stimulate the laryngeal mucosa and also stimulate the distal esophagus and stimulate the vagus nerve, causing chronic cough and throat clearing, thus causing damage to the mucosa of the vocal cords, resulting in a series of symptoms: pharyngeal foreign body sensation or hysterical ball sensation, hoarseness and dysphonia, chronic cough (mostly irritating dry cough), throat clearing, sore throat, dyspnea, bad breath, pharynx The first three of these are particularly common. Some patients have typical GERD symptoms: acid reflux, heartburn, chest pain, dysphagia, abdominal distention, hiccups, and in a few cases, pulmonary symptoms such as shortness of breath.  Currently, the diagnosis of reflux pharyngitis relies on typical symptoms, fiberoptic endoscopy (laryngoscopy, esophagogastroscopy), 24-hour esophageal PH pressure monitoring, and other tests. Diagnostic treatment is available for some highly suspected patients (1 to 2 weeks of treatment with acid suppressants and improvement of symptoms can confirm the diagnosis).  The treatment of reflux pharyngitis is mainly pharmacological treatment combined with lifestyle changes: 1. pharmacological treatment includes: gastric stimulants (e.g., dopantridone), acid suppressants (e.g., omeprazole), mucosal protective agents, etc. (2-3 months); 2. lifestyle changes include: obese people should lose weight, reduce fatty meals, avoid overeating, sweets, spicy foods, acidic fruits (oranges, prunes, etc.), quit smoking and alcohol Strong tea and coffee, especially before bedtime should be reduced to drink hot tea, drinks, 3 hours before bedtime do not eat, can reduce the night gastric acid secretion. Postural therapy is also an important means of treating the disease, such as maintaining an upright position after meals, avoiding bending over to pick up things, avoiding excessive weight bearing, and elevating the head of the bed during nighttime sleep. Adjust your mind and keep a happy mood.  The duration of medical treatment requires more than 3 months, and it usually takes 2 to 4 weeks to show the effect.  Studies have shown that laryngopharyngeal reflux may be associated with laryngeal cancer, esophageal adenocarcinoma, secretory otitis media, laryngeal stenosis, laryngeal wheezing, chronic rhinosinusitis, obstructive sleep hypoventilation syndrome, dental enamel damage, and infant death syndrome. Therefore, if you have similar symptoms, you must go to a regular hospital for appropriate examination and treatment. With a clear diagnosis, such diseases can be effectively treated.
