A 29-year-old male patient with low back pain with hunchback for more than 10 years, normal urination and defecation. On examination: the thoracolumbar segment showed lateral kyphosis, activity was obviously restricted, muscle strength and sensation of both lower limbs were basically normal, physiological reflexes existed, pathological reflexes were not elicited, and sphincter function was normal. The imaging data showed that the whole spine (thoracic 1-sacral 1) was about 60 degrees of kyphosis, the thoracolumbar segment was 35 degrees of kyphosis, the lumbar vertebra was 15 degrees of kyphosis, the cervical 7 plumb line was located about 10 cm in front of the posterior superior angle of sacral 1, the sagittal position was severely imbalanced, and the coronal scoliosis was about 20 degrees. Four days after surgery, the patient started to walk on the ground, and the radiographs showed that the lateral kyphosis deformity was basically corrected, the full kyphosis was about 10 degrees, the thoracolumbar segment was close to 0 degrees, the lumbar spine changed from posterior convexity to anterior convexity of 45 degrees, and the sagittal balance was basically restored.
