High myopia macular degeneration is a group of diseases that seriously affects vision, and there is no specific method to stop the development and progression of this pathology. The main pathological changes include macular hemorrhage, macular neovascularization, and posterior scleral chylomegaly. Treatment for neovascularization: 1.Laser treatment: The heat energy generated by laser is used to destroy the abnormal neovascularization in the macula. Laser photocoagulation only aims to close the existing neovascularization, and cannot prevent the formation of new neovascularization, it is a symptomatic treatment. However, a slight overdose of laser can itself cause choroidal neovascularization and damage to the nearby normal tissues, and the visual function will be greatly affected, so we must be vigilant. Also, the laser should not be too close to the macular recess to avoid permanent and irreversible damage to the vision. This laser is very limited in the treatment of macular degeneration. 2.Intraocular drug injection Lucentis: In recent years, intraocular injection of razumab (Lucentis) approved by FDA has achieved good therapeutic effect in the treatment of macular degeneration.Lucentis has been marketed in the U.S. in 2006, and was named as the top ten health news in the U.S. that year. It is a vascular endothelial growth factor (VEGF) fragment antibody that inhibits multiple VEGF isoforms, and many clinical trials have demonstrated its excellent therapeutic effect in the treatment of macular degeneration. 80% of the patients had varying degrees of improvement in vision. It is currently one of the best methods in the United States. Intraocular injection of anti-VEGF drugs for macular degeneration is currently one of the most important and effective means of treatment, and the main drugs are avastin and Lucentis. What is the difference between them? (1) avastin is an anti-tumor-based drug, which is used as off-label drug for intraocular injection of macular degeneration in foreign countries, the main feature is that it is cheaper. Charges vary from hospital to hospital, ranging from about 2,000 to 3,000 yuan. One of the biggest risks of doing this is that the chances of intraocular infections are greatly increased. The mass infection of more than 60 people in Shanghai First People’s Hospital the year before last was related to this operation. (2) lucentis is an intraocular injection of ophthalmology specifically for macular degeneration, a special package for ophthalmology dose, one person, so the chance of infection is greatly reduced. But its cost is expensive is the main problem, about $2000 in the United States. I hope the price of this drug will be reduced gradually. (3) From the point of view of drugs and effects, lucentis has better permeability in the eye and more obvious effects. 3, transpupillary thermotherapy (TTT): This method is the use of 810nm wavelength of near-infrared laser, the radiation rate in the retina is 7, 5W/cm2, strong penetration and refractive interstitial absorption less, so that the target tissue slowly warming about 10 ℃. However, it is lower than the local temperature produced by conventional laser photocoagulation, which acts non-specifically on the CNV and causes less damage to the surrounding normal tissues. After treatment, thrombosis within the CNV as well as partial or total CNV closure occurs, and the absorption of bleeding and exudation is promoted, while still relatively preserving some visual function. However, the determination of temperature is difficult and the results of different studies are sometimes contradictory. Nowadays, it is basically not used or eliminated. 4. Photodynamic therapy (PDT): It is a specific photosensitizer injected into the patient’s blood, and when the drug circulates to the retina, the photosensitizer is excited with 689 nm laser irradiation, thus destroying the abnormal neovascularization without damaging the normal retinal tissue. It is used to treat CNV in the subcentral recess, but the latest findings suggest that visual acuity is not improved as much as with intraocular injections of the drug. The results of studies in the United States and Hong Kong point to poor long-term results. Treatment for posterior scleral staphyloma – posterior scleral reinforcement. This is a relatively old method, almost forgotten by ophthalmologists and patients in recent years with the introduction of refractive surgery, which has been effective in stopping the formation and progression of and posterior scleral staphyloma. However, in the current medical system and atmosphere, doctors are also reluctant to perform this procedure, which is low-charging, invasive, and does not have immediate results.
