Abstract: The patient had recurrent dizziness and headache for more than 1 year, with a recurrence of 1 week, and was previously diagnosed as “hypertensive disease”. After treatment, the headache and dizziness were significantly relieved and the blood pressure was stable.

I. Initial consultation
The headache and dizziness were diagnosed as “hypertension” in the local hospital for more than 1 year, and the symptoms improved after the administration of amlodipine besylate tablets to lower the blood pressure and other symptomatic treatments. With weakness of the limbs, frequent panic attacks and chest tightness, sometimes nausea, poor sleep, normal appetite, normal bowel movements, dark red tongue, thin white fur, sunken string pulse. Examination: blood pressure: 170/110 mmHg, flushed face; auxiliary examination: carotid ultrasound: atheromatous plaque formation in the right common carotid artery and carotid bulb. Diagnosis: Western medicine diagnosis: hypertension grade 2, carotid plaque; Chinese medicine diagnosis: headache – hyperactivity of liver and Yang.
II. Treatment history
Initial diagnosis: the TCM diagnosis was hyperactivity of liver yang. The treatment was based on pacifying the liver and submerging yang, and on resolving blood stasis, with the prescription of Chinese medicine: tianma, hooked vine, cow’s knee, stone cassia, raw dioscorea, nightshade, gardenia, dulcimer, scutellaria, mulberry, wu wei zi, salvia, red peony, etc.
Re-examination: 3 days later, head swelling and pain slightly relieved, but the face flushed, night sleep improved, blood pressure was still high at 165/115 mmHg, after informing the patient of the need to lower blood pressure, oral amlodipine benzoate tablets were given to lower blood pressure, and continued to take the front.
Third consultation: dizziness, headache relief, blood pressure is basically normal range, but poor appetite, weakness is obvious, the tongue is dark red, thin white moss, the edge of the teeth, thin pulse, consider the spleen deficiency, damp stagnation, to benefit the spleen to resolve phlegm, the prescription: ginseng, Poria, Atractylodes, licorice, Astragalus, Huai Shan yam, sand, heliotrope, Perilla.
Fourth consultation: the patient’s symptoms basically disappeared, headache was completely gone, weakness was relieved, and food intake improved.
III. Treatment effect
The patient was diagnosed with hypertension and carotid plaque after admission to the hospital, and the diagnosis of Chinese medicine is headache – hyperactivity of liver yang. This can lower blood pressure, lower lipid and stabilize plaque. Therefore, the patient’s symptoms of dizziness and headache improved significantly, and the weakness was relieved, and the appetite improved after the herbal identification treatment.
Note
We are glad that the patient’s symptoms have improved after treatment, but we still need to remind the patient that there are many causes of hypertension, such as alcoholism, spicy food, fatty and sweet food, and emotional disorder, all of which can cause pathological changes such as loss of liver drainage, hyperactivity of liver yang, and internal disturbance of phlegm, leading to the occurrence of hypertension. For the initial increase of blood pressure, we can also take tea such as chrysanthemum tea, stone cassia tea, corn husk tea, etc.; eat celery, seaweed, potatoes and other foods; for the case of hyperactive liver yang, we can also eat steamed turtle to calm the liver and submerge the yang.
V. Personal insight
At present, the trend of hypertension is young, the treatment not only needs to regulate the liver and kidney, but also needs to be more important. This requires patients, especially young patients, to pay attention to regulating their emotions, improving their ability to conform to society, avoiding unnecessary tension and anxiety, arranging work and rest time reasonably, and eating a reasonable diet in order to maintain blood pressure balance. For patients with poor long-term blood pressure control, they should go to the hospital as soon as possible for standardized and systematic antihypertensive treatment to avoid a series of cardiovascular and cerebrovascular diseases.
