Gallbladder cancer has no specific clinical manifestations in early stage or only symptoms of chronic cholecystitis, which makes early diagnosis very difficult. Once persistent pain, masses and jaundice in upper abdomen appear, the lesion has reached advanced stage and its various examinations also appear abnormal. Therefore, patients with discomfort or pain in the gallbladder area, especially middle-aged and elderly patients over 50 years old with gallbladder stones, inflammation and polyps, should undergo regular ultrasound examinations for early and clear diagnosis. 1.Pain in the right upper abdomen Most of them are persistent pain in the right upper abdomen, and may have paroxysmal intensification, radiating to the right shoulder and lower back. This symptom accounts for 84%. Since gallbladder cancer mostly coexists with gallbladder stones and inflammation, the nature of pain is similar to that of stone cholecystitis, which starts with discomfort in the right upper abdomen, followed by persistent vague or dull pain, sometimes with paroxysmal severe pain and radiating to right shoulder. 2. Gastrointestinal symptoms The vast majority (90%) suffer from indigestion, aversion to grease, belching, and reduced gastric appetite, which is due to the inability of the gallbladder to renew its function and digest fatty substances. Nausea and vomiting are also quite common, and there is often loss of appetite. 3.Jaundice Due to the spread of cancer, about 1/3 to 1/2 of the patients develop jaundice. In a few patients, jaundice is the first symptom. Most of the jaundice appears after pain, and the jaundice is persistent and progressively worsening, while a few patients show intermittent jaundice. Jaundice tends to appear late in the course of the disease, accounting for 36.5%, mostly due to cancer tissue invading the bile ducts and causing malignant obstruction. It is accompanied by emaciation, weakness and even cachexia, yellow staining of skin and mucous membranes, and untreatable itching of skin. 4.Chill and fever Mostly appear in the late stage of cancer, 25.9% of patients have fever, and high fever may persist. 5.Mass in right upper abdomen When the lesion develops to advanced stage, a mass appears in right upper abdomen or upper abdomen, accounting for 54.5%. Firstly, the tumor grows rapidly and obstructs the bile duct, making the gallbladder enlarged; secondly, the obstruction caused by invasion of duodenum, and the obstruction symptom appears at the same time; in addition, if the liver, stomach and pancreas are invaded, the masses in corresponding parts may also appear.
