Recognizing myofasciitis

  Fasciitis of the low back muscles is known as low back muscle strain, low back fibrositis, low back fascial pain syndrome, rheumatism, etc.  Causes: The muscles of the low back are surrounded by fascia, and there is fatty tissue under the fascia, which can pass through the fascia to form a hernia. Nerve branches and blood vessels also penetrate the fascia. Injury to the muscle exudates and bleeds into the inflammatory adhesions between the muscle and fascia, exposure to moisture and cold, chronic strain on the muscle, hypertrophic adhesions of the fascia, compression of the neurovascular strangulation, strain on the fascial stopping point of the muscle, terminal disease lesions, acute lumbar back muscle injuries that are not cured in time to become chronic, and so on are the causes of symptoms.  Symptoms and diagnosis: 1. Low back pain is heavy when waking up in the morning, but improves after activity, and the symptoms are lighter during the day and aggravated at night, and some of them also affect sleep. It decreases or disappears when working or training, and low back pain when resting.  2.The pain changes with the weather, and the pain increases on cold or rainy days.  3.It is often difficult to work by bending over, and the pain increases when bending over for a long time.  4.The range of low back pain is wide and the pain is indescribable, such as hidden pain, swelling, soreness, and some are accompanied by heavy feeling.  Examination: Good range of spinal movement, forward bending lumbar pain, pressure pain, palpation may have muscle spasm, sometimes spastic muscle strips or hard nodes can be palpated, pressure pain is obvious, and even radiating pain. The pain disappears with local anesthetic closure. Full flexion pain and resistance pain of the low back muscles.  Differential diagnosis Should be differentiated from disc herniation, ankylosing spondylitis, tuberculosis of the spine, metastatic lesions of the spine (e.g., tumors), and osteoarthritic lesions.  Treatment conservative treatment: control the amount of exercise, proper exercise of the lumbar back muscles, after exercise and at night before going to bed to do pulling exercises of the lumbar back muscles, i.e. body forward flexion to try to elongate the lumbar back muscles, which can relieve muscle spasm and improve blood circulation.  Physical therapy: such as heat therapy, ultrashort wave, short wave, cupping, etc. Massage and acupuncture are effective.  Oral medication: such as vitamin E, fotarine, pau taisong, fenpropathrin, loxoprofen, etc.  Topical medications: Chinese herbal medicines to relieve pain and activate blood circulation are also effective.  Local injection: glucocorticoids or injection of hyaluronidase + lidocaine hydrochloride is effective.
