Menopause syndrome is also known as “the evidence before and after menopause” in Chinese medicine. It is mostly due to the gradual decline of the innate kidney qi, the deficiency of the Ren pulse, the failure of the Taichong pulse, and the exhaustion of the Tianjue, resulting in the imbalance of the body’s yin and yang, or the deficiency of kidney yin and the failure of Yang to hide; or the deficiency of kidney yang and the failure of the meridians to warm and nourish, resulting in a series of symptoms of disorders of the internal organs.
I. Etiology and pathogenesis
The disease is mostly caused by old age, weakness of kidney energy or the influence of factors such as childbirth, mental and emotional, so that yin and yang are out of balance, causing disorders of the heart, liver, spleen, kidney and other internal organs. Liver and kidney yin deficiency, Yang loss of subtlety and hyperactivity is the main pathogenesis of this disease.
Yin deficiency of the liver and kidneys is caused by Yin deficiency of the body, or loss of blood and fluid, and multiple childbirths due to room strain, resulting in deficiency of kidney qi, lack of essence and blood, inability of kidney essence to transform blood, insufficient source of liver blood, and lack of water to contain wood, resulting in Yin deficiency of the liver and kidneys.
The heart and kidney do not intermingle because of the deficiency of liver and kidney, kidney water is not enough to help the heart, the heart fire is too vigorous to descend to the kidney, the heart and kidney do not intermingle, the god is not nourished and see this evidence.
Qi stagnation and blood stasis Mostly due to narrow-mindedness, mood disorder, anger and depression, resulting in liver qi stagnation or qi imbalance and qi stagnation and blood stagnation, which leads to various pathological phenomena of liver and blood stagnation.
Spleen and kidney yang deficiency The evidence of spleen and kidney yang deficiency occurs when there is a deficiency of yang in the body or a prolonged illness and kidney or excessive exertion, which damages the kidney yang, and the kidney yang is insufficient to warm the spleen yang.
Identification and typing
In clinical diagnosis, we mainly identify the deficiency of the internal organs and seize the common feature of deficiency of kidney qi, and identify the evidence of concurrent evidence to distinguish the types of evidence. The following five types are summarized according to the modern identification of various schools of thought.
1, liver and kidney yin deficiency dizziness and tinnitus, irritability, baking heat, sweating, palpitations and sleeplessness, forgetfulness, five heartburn, spasms and weakness of the waist and knees, menstrual cycle disorders, menstrual volume or less or dripping, bright red color. The tongue is red with little coating, and the pulse is thin and stringy.

3.Liver Qi depression, depression, dysthymia, breast pain or tingling around the body, dry mouth, bitter mouth, sighing, menstruation or before or after, menstrual flow is not smooth, abdominal distension and pain, sadness and crying, suspicious and worrying, short urine with red color, dry stool. The tongue is red, the coating is yellow and greasy, or the tongue is blue or petechial, and the pulse is stringent or astringent.
4.Spleen and kidney Yang deficiency, menstrual disorders, large amount of light color, cold form and limbs, tiredness and weakness, dull face, floating and swollen skin, waist and knee cold, abdominal fullness and poor taste, loose stools. The tongue is tender, the fur is thin and white, and the pulse is sunken and weak.
5.Yin and Yang deficiency of the kidney. Red cheek and lips, sleeplessness, hot flashes and night sweats, dizziness, tinnitus and palpitations, sensitivity and irritability, cold body and limbs, soreness and weakness of the waist and knees, closed menstruation, decreased sexual desire. The tongue is pale and the pulse is sunken and weak.
III. Fractal treatment
(1) Liver and kidney Yin deficiency
Treatment: Nourishing the liver and kidney, nurturing Yin and submerging Yang.
Prescription: 15 grams each of raw earth and yam, 12 grams each of Fructus Lycii, Fructus Chasteberry, Fructus Cornu Cervi Pantotrichum, Fructus Paeoniae, Fructus Shou Wu, 10 grams each of Dampi, Poria and Zeligia, 30 grams each of Shenglongmu (first decoction).
Add and subtract: add mother-of-pearl for high blood pressure; add Chuanjian and Sangxiang for lumbar pain; add nightshade vine and helianthus bark for insomnia.
How to use: 1 dose daily, decoction in water, divided into 2 doses.
(2) Heart and kidney disconnection
Treatment: Nourishing Yin and lowering fire, and promoting intercourse between the heart and kidney.
Prescription: Huang Lian, Scutellaria, Licorice 6g each, Bai Shao, Jujube 15g each, Agaricus (melt and punch), Lily, Zhi Mu 10g each, Chicken Seed Yellow 2 pieces, Cinnamon 3g.
Add and subtract: add dragon bone, oyster, magnet; add amber and lotus seed heart for forgetfulness and dreaminess.
How to use: 1 dose daily, decoction in water, divided into 2 doses.
(3) Liver Qi Stagnation
Treatment: Diversify Liver-Qi, clear heat and nourish Yin.
Prescription: 12 grams each of Radix Bupleurum, Atractylodes Macrocephala, Poria, Radix Paeoniae Alba, 9 grams each of Radix Angelicae Sinensis, Dampi and Uliphanthes, 6 grams each of Rhizoma Chuanxiong, Pericarpium Citri Reticulatae, Radix Glycyrrhiza Uralensis and Peppermint.
Addition and subtraction: bitter and irritable mouth plus scutellaria, gardenia, gentian grass; blue and purple tongue with ecchymosis plus peach kernel, safflower.
Usage: 1 dose daily, decoction in water, divided into 2 doses.
(4) Spleen and kidney Yang deficiency
Treatment: Warming the spleen and kidney.
Prescription: 6 grams each of gunpowder and ginger, 12 grams each of ginseng, atractylodes, poria, poria, zedoary, and 6 grams each of roasted licorice.
Addition and subtraction: add yam, lentils for loose stools; add rehmannia, shamrock, cinnamon for kidney yang deficiency.
Usage: 1 dose daily, decoction in water, divided into 2 doses.
(5) Deficiency of both kidney yin and yang
Treatment: Benefiting kidney yin, warming kidney yang, dipping deficiency fire, and regulating flushing.
Prescription: 10 grams each of Xianmao, Xianling spleen, Bacopa monniera, yam, prunus seed, lady’s mantle, Semen Cuscutae, mulberry seed, 18 grams each of Shu Di and Shou Wu, 9 grams of Zhi Mu.
Add and subtract: insomnia and panic attacks with Shehuan Pi and Lily; restlessness with Dragon’s tooth and oyster.
Dosage: 1 dose daily, decoction in water, divided into 2 doses.
