This is a true picture of our orthopedic ward five patient Han. Han, 25 years old, is a resident of Gagdalchi Forest District. Seventeen years ago, he was diagnosed with ankylosing spondylitis in Beijing due to poor spinal joint pain and movement. For Han’s family this is undoubtedly a bolt from the blue, since then began a long process of seeking medical treatment, large hospitals, small clinics, and even black clinics have left their family footprints. They spent a lot of money, but the disease continued to develop, and the bilateral hip joints gradually fused completely and lost their functions. He was unable to stand, unable to sit, unable to walk freely, unable to take care of himself, his quality of life was seriously low, and he had lost confidence in life. After being introduced by an acquaintance who had been treated at the Fourth Hospital of Harbin Medical University, Han’s family came to the Fifth Ward of Orthopedics Department of Harbin Medical University with a faint hope. Professor Lian Yongyun, director of the fifth orthopedic ward, and Dr. Li Hongxi, attending physician, received the patient and conducted a detailed examination. It was found that the patient’s bilateral hip joints had completely bony fusion without gaps, the spine had no mobility at all except for the cervical vertebrae, and the fracture of the right femur near 2 segments was in a deformed healing state; worse still, the patient’s perennial hypertension had not been treated systematically, causing serious damage to the heart and kidneys, with an admission blood pressure of 200/120 mmHg, a heart ejection fraction of only 50%, and a renal ultrasound showing diffuse hyperplasia of both kidneys.  The medical team attached great importance to the patient’s condition and considered by the consultation of relevant departments to control blood pressure and improve cardiac and renal function, and at the same time formulated the surgical plan of bilateral simultaneous total hip arthroplasty for the patient. The surgical design was very difficult. During the operation, each side of the joint replacement only takes about 1 hour, which is short and the patient bleeds less. This one-stage bilateral surgery shortens the hospital stay for the patient and greatly reduces the economic burden. After the surgery, the rehabilitation and nursing team of the Department of Orthopaedic Surgery gave the patient the most professional rehabilitation guidance and treatment in a timely manner, so that the patient recovered quickly after the surgery. One week after the operation, the patient could already get out of bed and walk with weight with the help of crutches, and could sit and stand on his own, allowing the patient to sit and eat again for 17 years and to take care of himself. Two weeks after the operation, the patient successfully removed the stitches and was discharged from the hospital.  Director Lian Yongyun introduced that ankylosing spondylitis (AS) is an autoimmune disease in which the body develops an abnormal immune response, causing damage to the sacroiliac joints and the spine, as well as affecting various organs and muscle tissues to varying degrees. The impact on peripheral joints, mainly the hip and knee joints, is obvious, and in severe cases, hip fusion and loss of motor function occur. As AS patients are often very young, due to the disease, they lose their ability to live normally and lose their dignity, which brings great impact to their families and themselves. At present, artificial joint replacement is the only effective solution. By reconstructing the joint and restoring joint mobility, patients can live and work normally, thus bringing great confidence and faith in life to the whole family and the individual. However, such patients are weak, have poor organ function, bleed a lot and are prone to infection, while the fusion of the hip joint is not conducive to intraoperative operation, which poses a great test for surgery and subsequent treatment.
