Fu Zheng Method for Ankylosing Spondylitis

The following is a brief summary of his experience in the treatment of ankylosing spondylitis (hereafter referred to as AS) for the benefit of fellow patients. 1, the understanding of the etiology of ankylosing spondylitis – kidney deficiency and cold, phlegm and stasis obstruction Ankylosing spondylitis is a systemic disease of unknown origin, with chronic inflammation of the medial joints as the main cause. It involves almost all of the sacroiliac joints and often results in calcification and bony ankylosis of the intervertebral discs and adjacent ligaments. Modern medicine often uses non-steroidal anti-inflammatory drugs, glucocorticoids and slow-acting drugs such as sulfasalazine, methotrexate, azathioprine, etc., which can improve joint pain to some extent, but lack effective treatment for the final ankylosis of the joint. Although the name “ankylosing spondylitis” does not exist in Chinese medicine, there are many descriptions of the characteristics of the disease, such as the “Nei Jing”, which describes “the tail of the spine as a substitute for the heel and the spine as a substitute for the head”, as well as the “ankylosis of the backbone”, which is scattered in many medical texts. In 1997, the Chinese National Standard “Terminology for the Treatment of Chinese Medicine” classified this disease as “spinal paralysis”, defined as pain in the lumbar spine, restricted movement of the two hips, or in severe cases, deformation of the spine or even straightening and stiffness; or back pain, muscle stiffness It is a paralysis disease that is aggravated by rainy days and exertion. After years of clinical treatment, Prof. Chen Xiangjun believes that the disease is most common in young men, and the invasion of the joints is mainly in the lumbosacral, middle spine, and cervical areas, where the circulation of the Governor’s Vessel is located, and the joint lesions are mainly localized stiffness, heaviness, and pain. The disease is caused by the combination of internal cold and external cold. Cold is stagnant, and damp is heavy and sticky, and both evils tend to condense phlegm into stasis, resulting in pain and stiffness in the spine, heavy and difficult to stretch. The loss of Qi and blood, the failure to transform cold and dampness, and the depletion of kidney yang, further lead to the formation of phlegm and stagnation, resulting in stiffness and pain in the joints, and even stiffness and deformation. Therefore, Dr. Chen advocates that the treatment of this disease must internally warm the yang of the spleen and kidney, externally dispel the yin of cold and dampness, and over time, cooperate with the method of activating blood circulation and resolving blood stasis, resolving phlegm and clearing the ligaments. For patients with heavy cold and dampness and obvious stiffness and pain, Dr. Chen advocates combined internal and external treatment, combined with external application of white mustard seed cake to strengthen the local warming and meridian-clearing effect. 2, the specific application of the Fu Zheng method for the treatment of ankylosing spondylitis: (1) the main external cold emphasizes warming and dispersing cold: the onset of the disease with obvious external cold and damp triggers, such as wind and rain, living in a cold and damp, cold to the touch, etc., the patient often manifests as lumbosacral heavy stiffness, stiffness and swelling in the spine, the limbs are not warm, the tongue is white and slippery pulse and other signs of cold and damp trapped Yang. Dr. Chen advocates that when the evil of cold and dampness first enters, the Yang of the spleen and the body should be encouraged to warm the meridians to get rid of cold and dampness. The treatment is commonly used to make Chuan Wu, Sheng Ma Huang, Sheng Huang Qi, Chuan Gui Zhi, Sheng Bai Zhu, Fried Fang Feng, Fried Bai Shao, and Fang Ji. The formula takes the meaning of Wu Tou Tang and Huang Qi Fang Wei Tang. The key to the treatment of stroke The fifth book on the pulse evidence and treatment of stroke said that “the disease can not be flexed and stretched, pain, wu tou Tang is the main”. You in Jing had cloud “cold dampness of the evil, non-ephedra, wutou can not go, and the disease in the tendons and joints, and non-skin hair of the evil can sweat out and disperse, so to Huangqi of the complement, white peony collection, licorice slow, hold the two things, so as to get deep and go to stay evil”. Chuan Wu a medicine, big pungent big hot, expel cold dampness effect of great magnitude, and open the couples, so that the cold dampness from the external attack from the table sweat and solve, and can warm the menstrual pain, for the treatment of cold dampness in the table, muscle and joint stiffness and pain of the first choice; raw ephedra, cinnamon, wind, Fangqi can also warm the cold, external dispel cold dampness; Huang Qi, white atractylodes is to strengthen the spleen Yang, support the Wei solid table for the important. Therefore, although the disease is mainly external cold, the treatment is still to help Yang first, but at this time the support of Yang more to Wei Yang, Spleen Yang is the main, help Yang at the same time to disperse cold and clear the ligaments, sweating to dispel dampness, the only proper treatment. (2) focus on warming the kidney yang: the disease has been a long time, the elderly, or with a variety of visceral damage, clinical often lumbosacral stiffness and cold pain, lumbar back and neck are unfavorable lateral rotation, fear of cold, nocturnal urination, loose stools, light fat tongue and sunken pulse weak and other signs of spleen and kidney Yang deficiency, serious cases can be accompanied by chest tightness, shortness of breath, swollen limbs and urine and other signs of the heart, lungs and kidney Yang Qi failure, Chen Shi believes that at this time, the Yang deficiency is mostly responsible for the production of internal cold The treatment should be mainly to warm the spleen and kidney, with the help of dispersing cold and removing dampness. The treatment should be based on warming the spleen and kidneys, accompanied by dispersing cold and removing dampness. The formula takes the meaning of Yang He Tang combined with Dou Wu Zai Sheng Tang. In this formula, Shu Di, Deer Horns, Mulberry, Dog’s Backbone, Du Zhong and Huai Niu Knee warm the kidney and strengthen the muscles and bones; Qiang Wu, Dou Wu and Hosi enter the kidney meridian to search the wind and cold out of the kidney; Radix et Rhizoma Phellodendron and Ginger combine with Cinnamon to relieve the wind and cold in the kidney meridian; White Mustard Seed warms the meridians and can dispel the phlegm outside the skin and membrane, so that the condensed evil can go out. The combination of several medicines can warm the kidneys and pass through the channels, dispel dampness and dispel cold. (3) To remove phlegm and invigorate blood throughout: Patients in both early and late stages of the disease can be accompanied by unfavorable flexion and extension of joints, muscle constriction, stagnation of tendons and veins, difficulty in bending the spine or even stiffness and deformation, and other signs of stagnation of blood and phlegm, so Chen advocates that the method of removing phlegm and invigorating blood should be used in the early stages of the disease, which can facilitate the removal of cold and dampness outside; secondly, it can promote the circulation of qi and blood and warm the yang energy, which can avoid the re-attack of cold and dampness and enable evil to The third is to facilitate joint movement and avoid stiffness and deformation of joints. If the joint pain is mainly stagnant, muscle numbness and tingling, accompanied by dark face, light purple tongue with petechiae, it is mostly blood stasis, and should be used to invigorate the blood and open the ligaments, Chen Shi commonly uses blood invigorating herbs such as Su Mu, cuttings, Yan Hu Suo, Chuan Xiong, Angelica, Red Peony, Curcuma, and Turtle worms; if the joint is stiff and swollen, swollen and deformed, and the tongue is white and greasy, it is mostly phlegm-damp, and should be used to resolve phlegm and open the ligaments. For those with muscle constriction and tendon stagnation, Chen also uses Ge Gen, Wild Papaya and White Peony to relax the tendons and activate the ligaments. (4) For patients with obvious local stiffness and pain, combined internal and external treatment is recommended: For some patients with obvious joint stiffness and pain and unfavorable rotation, Dr. Chen advocates combining external application of local herbal cakes with internal Chinese medicine. The medicine: raw white mustard seed, raw Chuan Wu, raw Nan Xing and ice chips are powdered together and mixed into a paste with equal amount of flour and appropriate amount of yellow wine to make a cake (the size depends on the diseased joint) and applied to the affected area with gauze compartment, supplemented by infrared, spectrum or microwave irradiation once a day for 15-30 min, 30 days as a course of treatment. The raw white mustard seeds, raw Chuan Wu and raw Nan Xing are all pungent and hot products in this formula, which can quickly warm and disperse cold and dampness, dissolve phlegm and activate blood when applied externally; plus the pungent fragrance of ice chips can help the medicine penetrate the skin and reach the disease. Infrared, spectrum or microwave irradiation can enhance local blood circulation, warm the blood, and strengthen the warming and dispersing effect of drugs. After external application, the cold and phlegm stagnation can be quickly dispersed, and the medicine taken internally can help to warm the Yang and disperse the cold, remove the dampness and open the ligaments.
