Be alert to wind, cold and dampness causing paralysis

  Paralysis is a common disease in rheumatology and corresponds to common clinical joint pathologies such as rheumatoid arthritis, psoriatic arthritis, osteoarthritis and gouty arthritis. Paralysis belongs to the category of difficult diseases, and the efficacy of western medicine is not satisfactory, and its toxic side effects have caused widespread concern. Traditional Chinese medicine has unique advantages in treating paralysis.  The classic work of Chinese medicine, the Yellow Emperor’s Classic of Internal Medicine, says in the chapter “Suwen Paralysis” that “the three mixed gases of wind, cold and dampness combine to form paralysis.” Dampness is the most important factor in causing paralysis and is related to the development and change of the disease throughout. “The spleen is the main source of dampness and is the central treatment. The weakened or impaired transport function of the spleen is the root cause of dampness in the body and the internal cause of external dampness invasion. The internal and external dampness resides in the central region of the body, the spleen, and the same qi is sought, resulting in paralysis is even more lingering and difficult to heal. Under physiological conditions, water and dampness or adhesions exuded from the joints can be dissolved and absorbed in a timely manner through the transport function of the spleen or transported to the internal organs through the transport function of the spleen, and finally discharged from the body to maintain normal nutrition and function of the joints. If the spleen is dysfunctional, causing the water and dampness transportation and qi-transformation is not effective, coupled with the dampness, so that these exudates, growths, adhesions can not be dissolved, absorbed, transported and dispersed and form swelling, cohesion, accumulation and even deformation of the joint tendons and flesh to cause disability. This is what the Su Wen Zhi Zhen Da Lun says: “All dampness, swelling and fullness belong to the spleen.”  The above is a discussion of the importance of the function of the spleen in the development of paralysis from the perspective of the causative factors and clinical manifestations of paralysis, and the following is an analysis of the pathogenesis of paralysis. “In other words, the main pathogenesis of paralysis is the blockage of blood and qi in the meridians and joints. In Chinese medicine, the root cause of the blockage of qi and blood is the deficiency of the spleen. The spleen is the “backbone” of the human body, the production of Qi, blood, fluid and the normal functioning of the internal organs, and the continuation of the body’s vital activities are dependent on the spleen’s transport function. The Yellow Emperor’s Classic of Internal Medicine says, “The spleen is the master of transportation and transformation, the blood is the master of muscles and limbs”. Modern medical research also shows that “spleen deficiency” can cause problems in cellular immunity, humoral immunity, low non-specific immunity and disorders in immune regulation mechanisms. Li Dong Yuan, one of the Four Great Masters of Jin Yuan, who is known as the representative of the “earth-supplementing school”, pointed out in his famous book “Treatise on the Spleen and Stomach” that “internal injury to the spleen and stomach causes all kinds of diseases”. In general, how the spleen functions in transport and transformation becomes the key to the treatment of this disease.  Based on this understanding of paralysis, we propose to regulate the spleen first and emphasize the importance of protecting the spleen and stomach while removing wind and dampness and activating blood circulation. First of all, the inclusion of spleen-regulating products in the treatment of paralysis formula is specific to the etiology and pathogenesis of paralysis. By reinforcing the function of the spleen, the individual’s constitution can be strengthened and the ability to resist disease can be improved, which is conducive to better efficacy of other drugs used to treat paralysis. When the function of the spleen is strengthened, dampness can be removed, qi and blood can be transported more smoothly, the limbs and muscles can be well moistened, and the symptoms of paralysis, such as swelling and pain, can be effectively relieved. This is the truth of the saying, “The matter is in the four directions, but the center is in the center”. On the other hand, the treatment period for paralysis is generally long, and many drugs can damage the spleen and stomach, leading to interruptions and half-heartedness. Non-steroidal anti-inflammatory drugs and hormonal drugs are the best in Western medicine for treating symptoms, but they are also the most common risk factors for gastrointestinal side effects, and long-term or heavy use can seriously damage the spleen. Therefore, taking care of the spleen and stomach is an important guarantee to complete the whole treatment process of paralysis. As the Yellow Emperor’s Classic of Internal Medicine says, “The spleen and stomach are the essence of the latter, and the source of biochemical energy and blood”.
