Overview and treatment of semen abnormalities

I. Overview Semen abnormalities include a decrease in total sperm count, low sperm motility and an increase in the number of abnormal sperm. In addition, excessive or low semen volume, excessive semen viscosity, non-liquefaction and abnormal acidity are also included as symptoms that affect the movement and survival of spermatozoa. It is the main cause of male infertility. Modern Western medicine, mainly using hormones or certain drugs, is not satisfactory. Modern application of acupuncture for the treatment of semen abnormalities was first reported in 1958. However, since then, until the 1970s, it was still almost mainly reported on a case-by-case basis. During this period, in terms of acupuncture points and acupoint stimulation methods, the traditional method was basically followed, such as the use of acupuncture points mostly selected from Kidney Yu, Guan Yuan, and Life Gate; the method was acupuncture or moxibustion, and the treatment time was longer, and some of them were accompanied by Chinese herbal medicine. In the last decade or so, larger samples of clinical observation articles have started to appear, and studies on the mechanism of acupuncture in the treatment of this condition have been conducted. In order to objectively assess the efficacy of acupuncture, the indications for acupuncture have also been explored. At present, the indications for acupuncture are: total sperm count < 0.6 billion/mL; sperm motility < 60%, deformed sperm > 15%. The efficiency of acupuncture is about 90%, and the efficacy of acupuncture for simple semen abnormalities is better than that for those with other conditions such as prostatitis. Foreign scholars have also affirmed that acupuncture has a more pronounced effect on sperm quality in men with low fertility. By observing several hormonal changes in male infertility patients treated with acupuncture, it was found that before treatment, the concentration of human chorionic gonadotropin (HCG) and testosterone (T) secretion in patients was lower than that of normal controls, and after treatment both rebounded to normal levels, suggesting that acupuncture can adjust the function of sex hormones and achieve the purpose of tonifying the kidneys and strengthening the yang and benefiting the qi and cultivating the yuan. It was also found that urinary 17-hydroxy and urinary 17-ketone, which were at low levels before treatment in these patients, increased after acupuncture, reflecting a significant effect on adrenal cortical function. II. Treatment Acupuncture (one) (a) Acupuncture points Main acupuncture points: in 2 groups. 1, Dahe, Qu bone, Sanyinjiao, Zhongji, Guan Yuan; 2, Bars, Kidney Yu, and Zhenmen. (ii) Treatment: Take 1 group at a time, alternating on alternate days. In the first group, the needles are first applied to Dahe, Qubone and Sanyinjiao, and after obtaining Qi, the light stimulation and tonic method is applied and the needles are retained. That is, in the Guan Yuan, Zhongji two acupuncture points to perform intermittent ginger moxibustion, moxibustion 3 strong, the size of the moxa cone to the moxibustion area appears red for the degree; the second group first needle eight s, after getting qi, moxibustion kidney Yu, vital gate, also for 3 strong. The needle is kept for 15-20 minutes, during which the needle is carried once every 5-10 minutes. 15 times is a course of treatment, and if it is not cured, the needle is stopped for 5-7 days and then the next course of treatment is continued. (C) efficacy evaluation The above method treated a total of 160 cases, according to similar to the aforementioned criteria assessment, 125 cases (78.3%), effective 33 cases (20.6%), invalid 2 cases (1.1%), the total effective rate of 98.9%. Among the 103 cases followed up, 80 cases were cured and the number of births was 68 (85%), while 23 cases were improved and the number of births was 6 (26%), indicating that the fertility rate of cured cases was high. Acupuncture and moxibustion (II) (a) Acupuncture points Main points: 2 groups: 1, Qihai, Guangyuan, Sanyinjiao; 2, Kidney Yu, Ji s, Taixi. Supporting points: Zhongji, Shusanli, Shouhai, and Zhanmen. (b) Treatment The main points are selected one group at a time, with two groups alternating in rotation. Use auxiliary points as appropriate. For the lower abdominal points, pierce straight or slightly downward, lifting and inserting, repeatedly exploring, so that the needle sensation is transmitted downward; for the back points, pierce straight for 0.8-1 inch, twist and turn to apply mainly the tonic method, and insert a 2 cm-long moxa section, ignite and apply warm needles. For Sanyinjiao and Taixi, the needle is pointed upward, and the right thumb is used to press the underside of the point so that the needle sensation is transmitted; for Sansili and Shouhai, the method of flat tonic and flat diarrhea is applied with direct pricking. Retain the needle for 15 to 20 minutes. After taking the needle, the abdomen and lower extremity points can be fumigated with moxa in turn according to the sparrow pecking method for about 15 minutes, to the extent of local flushing. Daily or every other day, 10 times for a course of treatment, rest 5-7 days between courses. (C) Evaluation of efficacy The criteria for judging efficacy: basically cured: total sperm count > 0.7 billion/mL; sperm motility rate > 60%, deformed sperm < 15%; effective: total sperm count and motility rate increased after treatment, deformed sperm decreased; invalid: no change in the above three items before and after treatment. A total of 256 patients were treated, 155 cases (60.5%) were basically cured, 57 cases (22.3%) were effective, and 44 cases (17.2%) were ineffective, with a total effective rate of 82.8%. Acupuncture and moxibustion (III) (a) Acupuncture points Main acupuncture points: 2 groups: 1, Guan Yuan, Foot San Li; 2, Kidney Yu, San Yin Jiao. Supporting points: Taixi, Baxi, Zhongji, and Blood Sea. (b) Treatment: Choose one group per day, alternating between Guan Yuan and Foot San Li, and Kidney Yu and Sanyinjiao. Guan Yuan is stabbed straight or diagonally downward by 1 to 1.5 inches, and Kidney Yu is stabbed diagonally in the direction of the spine by 1.5 inches. Foot San Li and Sanyinjiao are pierced directly by 1.5-2 inches, and each point is tended by twisting and turning, with the needles remaining in place for 15-20 minutes and moving once every 5 minutes or so. For Yang deficiency, moxibustion of Kidney Yu and Guan Yuan is added; for Yin deficiency, Tai Xi is added; for phlegm-dampness and blood stasis, Eight S, Zhongji and Blood Sea are added, and the diarrhea method is performed. Acupuncture and moxibustion once a day, continuous treatment for 25 days intermittent 5 days, 3 months as a course of treatment, general treatment of one to three outside the course of treatment. (C) efficacy evaluation The above method treated a total of 86 cases, 46 cases were cured, 23 cases were effective, 13 cases were effective, and 5 cases were ineffective. The total effective rate was 94.2%.
