Points to note on semen sample collection in assisted reproduction

When entering the IVF cycle, the sperm collection on the day of egg retrieval is very important, so it is necessary to remind you of the following: 1) Abstain from sex for 3-7 days before sperm collection or follow the doctor’s instructions for early sperm removal 2) Ensure sufficient sleep and rest 2 days before sperm collection, and eat moderately on the day of sperm collection 3) Sperm can be collected from a nearby hotel and sent to the laboratory within half an hour after sperm collection, the shorter the interval, the better. Pay attention to insulation, sperm is sensitive to temperature, 25-37 ℃ is appropriate. 4. We must pay attention to cleanliness, wash our hands before sperm collection and avoid touching the inner wall of the sterile cup. 5. If coital sperm extraction is required, the male partner must do a good job of local cleaning to avoid infection at the puncture site, as the female partner will be performing egg retrieval surgery on the same day. (Regular condoms have a strong spermicidal effect and are forbidden to be used!) 6. For those who have had difficulties in sperm retrieval before, those who are older than 50 years old, or those who are easily nervous on important occasions, it is highly recommended to freeze the sperm for backup to avoid the pain of surgical sperm retrieval. The cost of freezing is 900 RMB. 7.If sperm retrieval fails, those who have frozen sperm beforehand can use frozen sperm after thawing for in vitro insemination, depending on the quality of the thawed sperm. If there is no frozen sperm beforehand, it is necessary to perform surgical sperm retrieval by puncturing and extracting the epididymis or testicular tissue. 8. If sperm retrieval fails and surgical puncture is not considered, egg freezing can be considered, but the technology is not yet mature. There is a greater risk that the frozen eggs will break after thawing and become unusable. Frozen sperm is thawed on the day of IVF and is cultured with fresh sperm for use. Some of the frozen sperm will definitely die after thawing, and if the sperm is in poor condition, all of them may die and cannot be inseminated; clinical studies have found no significant difference between the fetus born after fertilization with frozen sperm and normal sperm. Surgical sperm retrieval is an invasive procedure that damages local testicular tissues in an irrecoverable way, and pain under local anesthesia is unavoidable. Under normal circumstances, the sperm quality level is: masturbation sperm retrieval > intercourse sperm retrieval > frozen sperm > surgical sperm retrieval.
